Job Title: Community Guide

Location: Chattanooga, TN

Salary Range: $45,000-$50,000 annual salary

Our team is passionate about making a positive impact in our community, and we are looking for a compassionate and organized Community Guide to join us in furthering our mission.

The Community Guide is responsible for interacting with and guiding participants toward accessing assistance for many of their immediate needs necessary for stability and self-sufficiency. The Community Guide will work directly with parents to assess their needs, connect them with appropriate resources and services, and track the services received. This role requires a compassionate individual with excellent organizational and communication skills who is passionate about helping families thrive. The Community Guide will play a critical role in helping families navigate their way toward stability and self-sufficiency.

Key Responsibilities:

  • Conduct comprehensive assessments to identify the needs of families.
  • Develop and implement individualized service plans.
  • Connect parents with community resources, such as essential needs, healthcare, housing, education, and employment services.
  • Maintain detailed and accurate records of services provided and track progress.
  • Assist families in navigating the necessary support and services.
  • Assist families in navigating relevant FTF services such as coaching, mental health services, education, etc.
  • Collaborate with other team members and community partners to provide holistic support.
  • Advocate on behalf of parents and their families to access resources and services.
  • Participate in team meetings, training sessions, and professional development opportunities.
  • Communicate with participants who have an individual service plan at a minimum of once per month (frequency is needs dependent).
  • Maintain current relationships with partner organizations.
  • Identify and establish relationships with relevant organizations and services.
  • Attend community resource fairs and networking events as opportunities arise.

Qualifications:

  • Bachelor’s degree in Social Work, Human Services, or a related field, preferred but not required with relevant experience.
  • Experience in case management, social work, or a related role is preferred.
  • Strong organizational and communication skills.
  • Passion for working with low-income families and helping them thrive.
  • Ability to work independently and as part of a team.
  • Knowledge of community resources and services.
  • Proficiency in maintaining detailed records and documentation.
  • Empathy, patience, and a genuine desire to support families in need.
  • Bilingual in English and Spanish preferred but not required.
